Outlined below are the Terms and Conditions that apply to treatments and interventions given by Sue Hinds to clients.
Definitions
Client
Client is defined as the person receiving the treatment.
Family / Carers
Family / carers may also be indirectly treated especially where the person is dependent on them for their day-to-day needs. This may include training and coaching services.
Consent to treatment
Consent to Treatment must be given by the client unless they are deemed unable to, i.e. lack capacity. See this NHS website: http://www.nhs.uk/Conditions/Consent-to-treatment/Pages/How-does-itwork.aspx
Consent may be withdrawn or withheld at any time by the client. I will always endeavour to check that the client is consenting to treatments. If someone is acting in the role of Deputy for a client the relevant documentation (e.g. Lasting Power of Attorney for Health and Welfare) must be shown to me.
Communication
This may be direct, by phone, letter, SMS or by email.
Payees
Payees are defined as the person settling the invoice if the client is not doing this directly.
Decision maker
Decision maker is the person who will receive the reports and recommendations and ultimately make the clinical decisions. This is usually the Client; however the Payee must give consent to any expenditure in terms of therapist input (time) or equipment.
Referral process
All referrals must be confirmed in writing by the client &/or payee and the written T&Cs agreed. This is usually accomplished via the relevant Consent Form.Referrals should include the following details:
– Full name and contact details of the client and payee (if different) including phone numbers and email addresses (if applicable).
– Who the Decision Maker is and who will receive reports / recommendations and in what form these are required, e.g. verbal, email, formal report.
– Confirmation that the Decision Maker is aware of the costs involved in the treatment and has the authority to pay invoices within the required period (this varies depending on the nature of the referral).
– The full nature of the disability/injuries and the specific request e.g. one off assessment to determine the requirements of the client.
– Supporting clinical documentation e.g. Medical, Case Management, Expert Witness, Accommodation or Equipment Reports.
– Recommended method of making contact and appointments withClient
– Contact details of other professionals involved with any relevant information or restrictions about contact with them.
– Any other information the referrer feels relevant.
On receipt of the referral, I will contact the relevant person to set up an initial appointment and will notify the Decision Maker of the same.
At the appointment I usually agree a recommended an initial action plan and timeline with the Client, possibly to be confirmed later with Payee / Decision Maker.
Report(s) may be written at this point depending on the agreed Terms at the Referral Stage.
Assistive communication equipment trials, visits to assessment centres, training recommendations and the like may be part of the recommendations. It is not possible to list all possible treatment/training recommendations in these Terms but the options will be given to the relevant people after the appointment(s).
Periodic reports and action plans (formal or informal) will be presented to the relevant people to keep them appraised as agreed at the time of referral and periodically throughout the intervention.
Payment
Fees vary depending on the type of work. Fees are payable by the hour / part-hour. Please refer to the relevant quotation sent to you by me.
One-off visits will usually generate a report with recommendations which will be accompanied by an itemised invoice for the work involved.
On-going interventions will be invoiced periodically depending on the nature and intensity of the interventions. All admin connected with the case is chargeable on an hourly/part hourly basis (e.g. phone calls, emails, research, liaison with other team members, meetings). Payment terms and instructions (including bank details) are clearly written on each invoice with a notice that a 10% surcharge per month will be levied for late payment of invoices.
Please indicate your preferred invoicing intervals when confirming instructions.
Change of payee must be notified to me immediately. Failure to do so will result in the original payee remaining responsible for settling subsequent invoices.
Cancellations may be chargeable depending on the reason for the cancellation and any costs involved to me in the cancellation. As a general indication: Cancellation (by client/carer) notice of:
– 7+days ahead, no charge unless preparations have already been undertaken, then hourly fee applies and any expenses
– 3-7days 25% fees payable
– 1-2days 50% fees payable
– Less than 24hrs – at my discretion
– Travel time and mileage will be charged for cancelled appointments if the session is cancelled on arrival at the appointment/ meeting
Data protection
All personal data I collect and process regarding the Client will always be kept securely and in line with the requirements of the General Data Protection Regulation. Please see my attached Privacy Statement for more details, which forms part of these Terms.
Complaints about my services
I hope that you will never have a need to complain about the treatment you are receiving. However, I am required by my professional body to notify you of my Complaints Procedure.
In the first instance you should approach me and discuss your concerns with me. You may do this verbally or in writing, whichever is best and easiest for you.
If you still have concerns you may contact the College of Speech and Language Therapists or the Health Professions Council for advice on how to handle and address your concerns.
If you have any questions or would like to discuss these Terms and Conditions before agreeing to them, by signing the relevant Consent Form and Payee Form (if relevant), please feel free to do so.
GENERAL DATA PROTECTION REGULATION – PRIVACY STATEMENT

Why we collect and process your data
We always ask for your consent to process your data in order to provide our specialist speech and language therapy services to you.
How and what information is collected about you
We may collect the following information by email, telephone call, by post, questionnaires / forms, or in-person:
– Name
– Date of Birth
– Contact details (phone, email, address)
– Family members / Support Staff names and contact details
– Letters of Instruction from Case Managers / Solicitors
– Medical history and lifestyle information, including reports and letters from other healthcare professionals
– School / College/Employer information (where necessary)
– Photographs / Videos (with your permission)
What we do with the information we gather and what it is used for
– Professional record keeping of client information;
– Recording the initial assessment;
– Provision of speech and language therapy services;
– Sharing your information with relevant parties when necessary.
Your rights
– If you have given your consent to us processing your data, you have a right to withdraw your consent to any further processing.
– You have a right to request your data to be erased from our records.
– You have the right to request your data be transferred to another organisation.

How you can access your information

There is no longer a charge for this service.
How long we keep your information for
The Regulation does not set out any specific minimum or maximum periods for retaining personal data. Instead, it says that: Personal data processed for any purpose or purposes shall not be kept for longer than is necessary for that purpose or those purposes. See this link
Security
We are committed to ensuring that your information is secure. In order to prevent unauthorised access or disclosure, we have put in place highly secure electronic systems and managerial procedures to safeguard and secure the information we collect. Sue Hinds will retain personal data on a secure electronic system, Qunotes.
Right to complain
